By accessing or using this Flores Memorial Medical Center (“FMMC”) appointment booking and EHR-enabled platform, you agree to these Terms & Conditions.

1. Eligibility and Account

• You must provide accurate and complete information.
• You are responsible for maintaining the confidentiality of your account credentials.
• You must not use the platform for unlawful purposes.

2. Appointment Booking

• Appointments are subject to doctor availability and hospital policies.
• Appointment details may be updated due to operational needs (e.g., schedule changes).
• Reminders are “best effort” and may be affected by network/provider delays or outages.

3. Notifications (SMS/Email)

• We may send SMS/email confirmations, cancellations, OTP, and reminders.
• You are responsible for ensuring your contact details are correct and accessible.

4. EHR Access and Use

Access to EHR information is restricted based on user roles and authorization. Any unauthorized access, disclosure, or misuse of medical information is prohibited.

5. Prohibited Activities

• Attempting to gain unauthorized access to the system or other users’ data
• Interfering with system availability, security, or integrity
• Submitting false, misleading, or harmful content

6. Limitation of Liability

The platform is provided on an “as is” basis. FMMC and the developers are not liable for indirect, incidental, or consequential damages, including missed reminders due to third-party delivery failures.

7. Updates to Terms

These Terms may be updated. Continued use of the platform means you accept the updated Terms.
